چکیده
Evaluating Off-Pump Coronary Artery Bypass Grafting To the Editor: We read with interest the article by Sedrakyan et al.1 The authors found that off-pump surgery was associated with a reduced risk of stroke, atrial fibrillation and surgical wound infections as compared with on-pump. If the results were presented as events, the reductions were translated into 10 fewer patients with stroke, 80 with atrial fibrillation and 40 wound infections per 1000 coronary artery bypass grafts using offpump. There were also 10 additional reinterventions with offpump surgery (median 1-year follow-up). This study was welldesigned and conducted, and it clearly demonstrated the limitations of randomized controlled trials that were included in this metaanalysis. We would like to emphasize where we stand about off-pump surgery, the quality of studies included in that metaanalysis and that we cannot easily generalize the conclusions. The sample size of the studies that were conducted in that scientific field were small, so the effect sizes and the power of these studies were also small. Confusion still exists about the clinical advantages of the off-pump approach. We need more strict criteria for defining the measured outcomes. It is clear that the flexibility in design, definitions and outcomes is great in these studies.2 The findings might be more precise when the outcomes are universally agreed (eg, death) rather than the situations where special definitions are made (eg, the definition of myocardial infarction is made only with rising creatine phosphokinase, new Q-wave or both, the definition of stroke as transient ischemic attack, permanent stroke or else). More high-quality randomized controlled trials (with adequate power) have to be conducted in this field. The majority of patients in these trials were low-to-moderate risk patients undergoing one or double-vessel bypass, which resulted in an overall low adverse event rate. This means that much larger studies and studies with high-risk patients with multiple vessel disease are required to prove that a difference exists between treatment strategies. High-risk patients with multiple vessel disease are supported to benefit more from the off-pump technique. The outcomes such as death, stroke, myocardial infarction, etc, must be examined in 1 and more years (3 and 5) with more events (for example at least 100 events). Caution must be exercised in interpreting the results because of selection bias, publication bias and time lag bias. We need randomized controlled trials and enhanced research standards to diminish bias.3

Avoidance of Dialysis in an End-Stage Renal Disease Patient Status-post Off-pump Coronary Artery Bypass Grafting
Certain benefits are clearly associated with the use of off-pump coronary artery bypass grafting (CABG) as compared with the on-pump CABG. The superiority is more evident in patients with multiple co morbidities including renal failure. We reviewed the medical records of a 67-year-old male with a past medical history that was significant for multiple cardiovascular diseases and new-onset end-sta...
متن کاملPrediction of Changes in Left Ventricular Ejection Fraction after Off-Pump Coronary Artery Bypass Grafting Surgery by Myocardial Perfusion Single-Photon Emission Computed Tomography
Introduction: Left ventricular ejection fraction (LVEF) is considered to be the single most important prognostic factor in patients with previous myocardial infarction. LVEF is not improved in all patients after coronary artery bypass grafting (CABG). This study aimed to assess the possibility of prediction of LVEF changes after CABG using myocardial perfusion gated signle photon emission compu...
متن کاملOff-Pump Versus On-Pump Coronary Artery Bypass Graft Surgery, prospective cohort study
Introduction: Coronary artery bypass grafting is a surgical operation that bypasses atherosclerotic coronary vessels and restores blood flow to the ischemic zone of hearts. And two different conventional methods are ON-pump CABG, and Off-pump CABG. Altogether, the efficacy and safety of these two types of surgical procedures are still under debate. In this study, we aim to com...
متن کاملEmergency Conversion from Off Pump to Cardiopulmonary Bypass in Patients with Coronary Artery Bypass Graft Surgery
Introduction: As off pump Coronary artery bypass grafting surgery (CABG) is a developing method in coronary cardiac surgery, most surgeons are anxious about the danger of emergency crash conversion. In this observational study we tried to show the rate and outcome of conversion . Material and methods: In this descriptive study about 477 coronary off-pump CABG patients were operated in Ghaem hos...
متن کاملHow I do it: high-quality intraoperative fluorescence imaging in off-pump coronary artery bypass grafting.
We have developed a simple technique for establishing high-quality intraoperative fluorescence imaging in off-pump coronary artery bypass grafting. The technique of transaortic injection of indocyanine green is an effective method of achieving clear fluorescence imaging and for evaluating the quality of graft anastomoses. We consider the images obtained with this technique to be equivalent to t...

Introduction: Acute kidney injury (AKI) is a common postoperative complication of cardiac surgery, which is associated with an increased risk of morbidity and mortality. This study investigated the frequency of postoperative AKI in low risk adult patients undergoing off-pump coronary artery bypass grafting (CABG).Materials & Methods: All consecutive adult patients of American Society of Anesthe...
